We require the following information in order for us to fill out a
Conveyance document for you

Your full name, address, telephone, fax numbers, and an email address, if you have one,
Email is the preferred way for us to communicate.

A copy (fax or email is acceptable) of a title report, title insurance policy or title document (such as deed or lease assignment) by which the present owner acquired the property and
that contains a legal description of the property being transferred (we can obtain the foregoing from a title company if necessary, for which there will be a
modest charge);

The full name, marital status, and address of the person or persons to whom title will be transferred;

The tenancy in which the new owners wish to hold title.

The amount, if any, which the new owner is paying to get title.

Whether an escrow company is involved, and if so, the name, address,
and contact information of the escrow officer.

Whether the property is to be conveyed subject to a mortgage. If so, please provide us with a copy of the mortgage, the loan number, the address of the mortgage holder and whether the mortgage is to be assumed.

If the property is a leasehold property, provide us with the name and
contact information of the Lessor so we can obtain consent or provide notice. (Normally, the lessor requires a payment for its consent.)

If the real property being transferred is registered in the Land Court system and the owner has died, married or divorced, we must petition the Land Court to note such fact on the Transfer Certificate of Title for such property. If this is the case, the client should provide us with certificate of death, marriage or divorce, as applicable.
